1. Do you have difficulty opening your mouth?
2. Do you have chronic headaches?
3. Do you have difficulty chewing certain types of foods?
4. Do you have ringing or popping sounds in one or both ears?
5. Do you have facial or neck pains?
6. Does your jaw feel sore at times or constantly?
7. Do you have difficulty turning your head in either direction?
